Make a Referral

Who can refer?

Young people may be referred to the project by a family member, school, social work, GPs, voluntary organisations and other agencies. Young person may also refer themselves to the project.

When we receive the referral, we will send information about the project out to the young person and their family, along with a consent form. The young person will then be assigned a key worker. They will contact the family or young person to arrange either a house visit or a meeting at school.

Referral form

The YCP aims to work with young carers in a way that reflects the principles of the United Nations Convention on the Rights of the Child. YCP staff can therefore meet with young carers without parental consent – but only during school time.
